Yes - I am a freelance patient and public involvement consultant. I have a contract with the National Perinatal Epidemiology Unit at the University of Oxford to be a co-lead on parent, patient and public involvement on their Policy Research Unit. I also work on research projects as a co-applicant for the National Childbirth Trust (NCT) and Nottingham Clinical Trials Unit, and am a member of the Public Advisory Board for Health Data Research UK.

c) Received honoraria: one-time payments (in cash or kind) from a related organisation?
Yes - Received a one-off payment from Nottingham Clinical Trials Unit for assisting with the set up and maintenance of a Facebook group for parents to discuss maternity research.

d) Served as a director, officer, partner, trustee, employee or held a position of management with a related organisation?
Yes - Former employee of the National Childbirth Trust.

e) Possessed share-holdings, stock, stock options, equity with a related organisation (excludes mutual funds or similar arrangements where the individual has no control over the selection of the shares)?
No
