a) Received research funding: any grant, contract or gift, commissioned research, or fellowship from The Cochrane Collaboration or a related organisation (i.e. any organisation related to health care or medical research) to conduct research?
Yes - -I am a Co-Director at Cochrane Australia, Monash University. In 2020 Monash University (my employer) received staff reimbursement costs of £3,000 and consultancy costs totalling £5,000 from Cochrane. This money was for statistical support provided by a Monash employee and for production of training materials. I was not included in the decision making process, the service was provided by a seperate unit to the one I lead, and all transactions were completed at arms' length. -My Centre (Cochrane Australia) was funded to lead Project Transform through Cochrane's game changer initiative (no personal funding) 2015-2019 -Australian National Health and Medical Research Council (NHMRC) funding for Cochrane Australia and number of related research projects -Cochrane Australia is a member of NHMRC Health Evidence Panel and so receives renumeration from NHMRC and Australian Department of Health for contract research and commissioned systematic reviews -Funding from Monash University, Department of Health and Australian Stroke Foundation to support projects related to Living Guidelines and Systematic Reviews -Cochrane Australia (no personal funding) received past funding from Cochrane in support of Task Exchange and Living Systematic Reviews

b) Had paid consultancies: any paid work, consulting fees (in cash or kind) from a related organisation?
Yes - -Salaried academic- Monash University -I receive an honorarium from Australia's National Health and Medical Research Council for committee fees on their Synthesis and Translation of Research advisory committee

c) Received honoraria: one-time payments (in cash or kind) from a related organisation?
No

Yes - Salaried professorial fellow, School Public Health and Preventive Medicine, Monash University
